As the driver of an SU WA vehicle, I agree:
*
1. To use any such vehicle in a responsible way, ensuring care is taken and the vehicle is kept in a good condition, including compliance with the Schedule of Duties below.
2. To notify your coordinator at SU WA of circumstances that will affect my eligibility as a registered driver, particularly any forfeiture of a valid Western Australian Motor Vehicle Driver's Licence.
3. To keep log books on the use by me of any such vehicle (log books are kept in vehicles).
4. To comply with the reasonable directions of an SU WA staff member so far as the use of any such vehicle is concerned.
5. To prevent any unauthorised person from using any such vehicle.
6. I understand and acknowledge that SU WA reserves the right to seek from me any excess payable on an insurance claim.
7. I understand that if the vehicle is damaged in anyway whatsoever while being used on any roads other than ‘formed roads’, then I will be liable for any damage resulting from the use of the vehicle, over and above the normal excess costs.
8. Complete the following page, indicating my competence as a driver of an SU vehicle, including towing.

1. Ensure the vehicle and associated equipment is cleaned inside and out
2. Ensure the vehicle has at least half a tank of fuel before being returned
3. Complete an inspection of:
• General condition - interior, exterior and operational
• Any damage
• Cleanliness
• Tyre pressure (vehicle and trailers)
• Internal accessories
4. Complete documentation:
• Log book completed with odometer readings prior to leaving and upon return and km’s vehicle was used
• Any damages are reported and Vehicle Incident Report (kept with log book) form completed
• Basic faults are identified and appropriate action is taken to report or remedy them
